Based on the Registrar's Circular No. 4 of 2006 and the 239th Universiti Putra Malaysia Management Committee Meeting (JPU), the rate charged for the replacement of a lost Matric Card / Staff Card is RM30.00.
If you lose your Matric Card / Staff Card, please go to the UPM Bursar Office Counter to pay a fine of RM30.00 and then to the Security Division Counter to make a loss report by completing the Loss / Damage Form OPR/BKU/BR01/HR.
Employee Passes are issued by the UPM Security Division to employees of outside companies who serve on the UPM campus. Applicants are required to complete the Working Pass Application Form OPR/BKU/BR01/EMPLOYEE PASS and enclose two (2) passport-sized photographs and a copy of the Identity Card. For non-Malaysian applicants, enclose one (1) copy of a valid passport.
A fee of RM5.00 will be charged to apply for an Employee Pass and payment must be made at the UPM Bursar Office Counter.
