SPECIAL VEHICLE STICKER (DISABLED)

Applicants must fill in the Application form Special Vehicle Stickers (Disabled) (OPR/BKU/BR01/PELEKAT04) and are required to bring along:-

(i) Copy of matric card or staff card or Identification Card.

(ii) Copy of a valid driving license.

(iii) A letter of authorization to use the vehicle owner of the vehicle if the vehicle does not belong to students (Students only).

(iv) Confirmation letter from the doctor (UPM University Health Center or Government Hospital).
